First Visit

If your first visit is for your older children coming to our practice, please take the time to send us a copy of the prior medical and immunization record.

Also fill out the  Pediatric Associates of Wellesley Medical History Form and bring it to the visit. It is helpful to review it with you.

Be sure to bring your current insurance card with you. This will help us expedite your insurance verification and help us get you seen as quickly as possible. When joining our practice you will be asked to review our Financial Policy and sign a few forms including:

• Consent for Use and Disclosure of Protected Health Information - HIPAA
• Patient Eligibility Screening Form for Vaccine for Children Program
• Pediatric Associates of Wellesley Medical History Form

Prenatal Visits

Pediatric Associates of Wellesley offers prenatal visits to expecting parents at minimal charge. These visits are scheduled at your convenience any day of the week and are designed to give the expectant parents the opportunity to come in and meet our staff, see our office, and sit down with the doctor of their choice to answer any questions before the baby arrives.

Hospital Affiliations

Our primary affiliation is with Newton Wellesley Hospital, where our Pediatricians round daily to see newborns and sick children. Should your child need more advanced care at a teriary hospital, we will refer to both Mass General Hospital for Children  and Children's Hospital Boston.
