Referrals

It takes about seven days (depending on volume) to process a referral.

It is your responsibility to know your insurance plan - what it does and does not cover (out-of-state, non-participating providers, etc.), also which services require a referral and which do not.

Please check with the specialist's office to make sure they accept your insurance.

Physicians do not process referrals; you must call the referral line.

In most cases, a referral can be back-dated up to 180 days.

When you call the referral line, please leave the following information:

• Patient Name (with spelling)
• Patient Date of Birth
• Patient PCP (Primary Care Physician)
• Patient telephone number
• Type of Insurance
• Insurance ID (include all letters and numbers)
• Specialist first and last name (with spelling)
• Specialty
• Specialist location
• Specialist telephone number
• Specialist fax number
• Date of appointment

Any omission of the above information will result in significant delay in processing your referral.
